Analyze clinical data to find gaps in care related to Stars, HEDIS and transitions of care, then reach out to recently discharged patients by phone, telehealth and in-clinic visits to coordinate follow-up and prevent readmissions. • ADN or BSN in nursing
• Active, unrestricted RN license
• 3+ years clinical nursing experience
• Transitions of care, quality improvement or managed care exposure
• Athena EMR and DataHub
