What you’d do

Provide case management to patients and families in an acute care hospital, overseeing discharge planning and utilization management for an assigned caseload. You’ll need

• BSN, or ADN with BSN within 4 years of hire
• Current RN license in the State of Michigan
• 2+ years acute care hospital case management experience
