What you’d do

Support patients with complex medical and psychosocial needs in a chronic care management program. Outreach patients by phone at least 20 minutes per month, manage portal tasks, connect patients to resources, and refer clinical issues to an RN. Full time. • Medical Assistant certification preferred or obtained within 3 months of hire
• BLS/CPR Certification for Healthcare Provider (AHA) required
• Patient outreach and care coordination
• Chronic care management experience
