What you’d do

Hospice RN case manager who assesses patients' physical, functional, psychological and environmental needs, sets eligibility for hospice admission and recertification, and develops and revises the plan of care. You’ll need

• Graduate of an accredited nursing school (diploma, associate or baccalaureate)
• Current RN license or NLC multistate license
• Current driver's license
• Hospice assessment and plan of care experience
