You’ll need

• 2+ years clinical experience in hospital, outpatient or community care management
• 2+ years experience with chronic medical conditions like diabetes, COPD, hypertension
• Knowledge of disease trajectories and symptom management
• Work remotely from U.S. states excluding some specified states
• Occasional travel for team meetings and company events
