Care Management Coordinator OhioRISE– Field – Must reside in West Region of Ohio

What you’d do

Coordinate care for OhioRISE members as a field-based teleworker covering Western Ohio, traveling to member homes and other locations up to 50% or more of the time. You’ll need

• 2+ years in behavioral health, social services, or human services
• 2+ years with PCs, keyboard, multi-system navigation
• MS Office Suite (Outlook, Word, Excel, SharePoint)
• Must reside in West Region of Ohio
• Travel within Western Ohio up to 50%+ of the time
