What you’d do

Registered nurses on the Transition Team help move clients from hospital care to ongoing home care in Chester, Delaware and Montgomery counties, delivering care and training the ongoing care team. You’ll need

• NICU, PICU, ICU, or home care trach/vent experience
• Passing score on high-tech adult and pediatric simulations
• Current CPR certification
• Ability to teach and educate the ongoing care team
