Provides medication management on the ambulatory care team, including individualized drug therapy, lab monitoring, and patient medication education. • Completion of ASHP-accredited PGY1 pharmacy practice residency
• 3 years clinical pharmacy experience in ambulatory care
• Protocol certification for collaborative drug therapy management
• Pharmacy residency preceptor experience preferred
