Support care managers delivering Tailored Care Management by doing outreach, engagement and follow-up with people who have I/DD, TBI, long-term services needs or pharmacy needs, helping them make informed choices about care and services. • 2 years of direct experience providing care
• Experience navigating the NC Medicaid delivery system
• Bilingual Spanish/English
