What you’d do

RN embedded in primary care practices who identifies and enrolls patients for complex care management, assesses them bio-psycho-social-spiritually, and builds multi-disciplinary care plans with PCPs, behavioral health, and pharmacists. You’ll need

• RN license
• 2+ years clinical experience preferred
• Care management experience preferred
• Experience with vulnerable patient populations
• Motivational interviewing
