Review patient charts and communicate with patients, families, and the medical team to determine whether patients meet acute rehab admission criteria. You’ll need

• LVN license
• 2+ years inpatient acute rehabilitation experience
• BLS Healthcare Provider from American Heart Association
• 5 years related experience in lieu of Bachelor's Degree
