Leads a team of Care Coordinators and Care Managers supporting children, adults, and families affected by abuse, violence, and trauma. • Bachelor's in Social Work, Counseling, Public Health, or related field
• Experience leading care coordination teams
• Licensure (LSW, LISW, LPC, LPCC) preferred
• Ability to build integrated care coordination models
