RN reviews clinical data in Athena EMR and DataHub to find care gaps tied to Stars, HEDIS and transitions of care, then does phone, telehealth and in-clinic outreach to patients and providers after hospital discharge. • ADN or BSN in nursing
• Active, unrestricted RN license
• 3+ years clinical nursing experience
• Transitions of care, quality improvement, managed care or population health exposure
