What you’d do

Provide inpatient social work case management for patients of all ages and their families. Conduct psychosocial assessments, coordinate post-hospital discharge needs such as home care, nursing home placement, equipment, financial assistance and community resources, and ensure decision makers are involved in treatment planning.
