What you’d do

Provide case management for LGBTQIA+ clients in need of housing support, including assessments, written care plans, and referrals to community services. • 1 year human services experience with high-risk or medically underserved populations
• Bachelor's in social science, public health, or healthcare administration preferred
• Case management and care plan development
