What you’d do

Serve as a liaison between patients, FHCSD staff, and outside providers to improve care and patient flow. Conduct phone and in-person interactions to give referrals, resources, and follow-up, and document all work per department, grant, and insurance standards. Full time. • High School diploma or GED equivalent
• 2 years of health education experience
• Preferably community health setting experience
• Certifications may be required or preferred depending on assignment
