• RN with 2 years direct clinical care, or unrestricted LPC/LMSW/LCPC/LCSW/LMFT license
• 2 years direct clinical care in a clinical setting
• State licensure (TX, NM, IL as applicable)
• Care management and health education experience
• Data gathering and regulatory reporting
