Pediatric Assistant Case Manager III, Healthy Young Minds

What you’d do

Coordinate outpatient care for young mental health patients, working with physicians, therapists, families, schools and community resources. Duties include tracking patient functioning data, registry entry, direct patient and family contact, and crisis intervention.
