The Clinical Liaison builds referral relationships, assists with intake and pre-admission screening, evaluates patient appropriateness for admission, communicates rehab options to patients and families, and coordinates with external case management. You’ll need

• Experience in clinical liaison or healthcare coordination
• Ability to perform clinical judgment for patient admission
• Strong communication skills with patients and families
• Experience working with case management and discharge planners
• Full-time schedule
