What you’d do

Provide in-home occupational therapy evaluations and skilled visits, building care plans focused on daily living activities, fall prevention, and functional independence. Train clients on adaptive equipment and home modifications, coordinate with nursing and the care team, and document in Homecare Homebase. Full-time.
